We performed autopsies in 72 cases of hepatocellular carcinoma (HCC) between 1989 and 1994 consisting of 45 cases (62.5%) of HCV-related HCC, 19 cases (26.3%) of HBV-related HCC, 4 cases (5.6%) of HCV and HBV-related HCC, and 4 cases (5.6%) of HCV- and HBV-negative HCC. All cases were examined clinicopathologically, and laboratory findings one month prior to death were analyzed and a comparison made. In terminal HCC, There were increases in AST and ALT Levels. AST levels increased more significantly than ALT levels. Levels of ALP, LAP, γ-GTP derived from the hepatobiliary tract significantly increased. The direct bilirubin level was 5 mg/dl and distinct jaundice was noted. Blood tests revealed anemia and reduced prothrombin time activity. The significance of differences was examined between HCV-related HCC and HBV-related HCC, showing a significant difference in albumin levels only. Histological findings showed that most cases were classified as being the nodular type, moderately-differentiated type or trabecular type in both groups. The incidences of metastasis were 73.8% in HBV-related HCC and 46.7% in HCV-related HCC. A history of blood transfusion was noted in a high propotion of patients with HCV-related HCC (p<0.05).
