Although there have been many reports of non-suppurative inflammatory myopathy, especially with respect to muscle fiber changes of polymyositis, details of the changes of the intramuscular peripheral nerve and motor end plates involved have lacking. We examined biopsied muscles of 10 cases by means of light and electron microscopy and by enzyme histochemistry. The clinical diagnoses were as follows : three cases of polymyositis ; one case of dermatomyositis ; one case of scleroderma ; four cases of polymyositis complicated respectively by Crohn disease, overlap syndrome, nodular goiter and Sjogren disease ; and a case of polyneuritis with collagen disease. In this report we make special reference to changes of intramuscular peripheral nerves, motor end plates and alterations in the muscle fibers themselves. Light microscopy showed changes in fiber type predominance, fiber type grouping, and fiber type deficiency. Collateral sprouting of intramuscular peripheral axons was revealed by methylene blue vital staining. The most significant and frequent changes found by the electron microscopy were empty folds of the end plates and "fibrous long-spacing collagen" (Luse) in the endoneurium of intramuscular nerves. The former changes were found in all seven cases examined. The latter findings were easily detected in 5 of the 6 cases, and suggested an abnormality of the tissue environment in peripheral nerves in this kind of disease process. The present authors believe that the neuronal changes in addition to muscle fiber changes are a morphological basis underlying a variety of symptoms including peripheral nerve disorders which frequently exist and prolongation of the clinical course in these disease entities.
