A case of von Recklinghausen disease with a submucosal leiomyomatous tumor of the duodenum is presented. Suggestive interest is referred to the difference from usual leiomyoma and neurofibroma in cases which are not associated with von Recklinghausen disease. The tumor was mostly composed of diffusely proliferated spindle cells, among which some eosinophilic globules were seen in hematoxylin-eosin stain. The spindle cells had the same character in actin-activity as smooth muscle cells and the globules were of collagenous fibers. Furthermore, the tumor showed significant character in quantity of collagenous fibers, more than leiomyoma and less than neurofibroma. This is interesting in the pathological point of view referring to the disease of tissue malformation, von Recklinghausen disease.
