Pancreatic endocrine function, especially insulin and pancreatic glucagon responses after an oral glucose load and intravenous glucose infusion, was investigated from the surgical point of view, and we attempted to study whether the hyperglucagonemia after total gastrectomy was due to the lack of stomach tissue or to truncal vagotomy. In the oral glucose tolerance test, plasma glucagon levels increased with intrathoracic gastric replacement after esophagectomy, interposition reconstruction (Billroth I method) after total gastrectomy, and especially in Roux en Y reconstruction (Billroth II method) after total gastrectomy. In the intravenous glucose tolerance test, there were significant differences in the insulin and glucagon secretion between pre-operative patients and post-operative patients. From those results we concluded that when truncal vagotomy was performed, whether stomach tissue was present or not, disturbances of carbohydrate metabolism due to hyperglucagonemia inclined to be brought out. In such a massive resection of the gastrointestinal tract as is required in truncal vagotomy, we could avoid a small degree of disturbance of carbohydrate metabolism by means of interposition reconstruction (Billroth I method).
