During the 8 years from 1975 to 1983,91 patients with carcinoma of the esophagus were admitted to our clinic. Resection with reconstruction in one stage was performed on 60 patients, with a resectability rate of 65.9 per cent. The hospital mortality rate was 8.3 per cent. The five-year survival rate was 7.2 per cent. The effect of the restored conduits on cardiopulmonary functions was investigated by monitoring with a Swan-Ganz catheter. The retrosternal route exerted more disturbing effect on cardiopulmonary functions than the posterior mediastinal one. However, the disturbed functions are expected to be normalized and the difference between the two routes disappears about four days after the operation. The result of resection suggests that the survival rate has a close relation to the extent of lymph node metastases. Since all the lymph nodes cannot be dissected, some immunochemotherapy should be combined in order to avoid recurrence after the operation.
