<Case Report>Intrahepatic cholangiocarcinoma associated with hepatolithiasis : a case report

A 66-year-old female with a mass in the left hepatic lobe was admitted. She had been suspected of having cholelithiasis ten years before because of an attack of chill and fever. The same attacks recurred recently. With a diagnosis of advanced intrahepatic cholangiocarcinoma, resection of the left hepatic lobe was carried out. Intraoperative ultrasonography and examination of the resected specimen revealed a cholangiocarcinoma associated with hepatolithiasis. Forty-four cases, including the present one, of co-existent lesions have been reported in the Japanese literature. All of the patients except for three male cases were in the 40's or older. The male-to-female ratio was equal with no significant difference in the average age between the sexes. Preoperative diagnosis of both lesions co-existent with each other was established in 9 cases. The left lobe was affected in 25 patients. Early hepatectomy for hepatolithiasis, especially a left lobectomy for a left lobe-limited one, is considered to be a preferable choice for avoiding the development of cholangiocarcinoma.
