A four-year old boy with corrected transposition of the great arteries (S, L, L, ) with ventricular septal defect and left atrioventricular regurgitation caused by Ebstein-like anomaly was successfully operated. Patch closure of ventricular septal defect through morphologic right ventriculotomy and left atrioventricular valve replacement with Bjork-Shiley prosthesis were simultaneously performed. Though the deformity, which is particularly like Ebstein's anomaly, of the tricuspid valve in the systemic chamber varies in morphology and severity, effective valvuloplasty is difficult in such a case with a severely deformed tricuspid valve as in our patient. Therefore we recommend valve replacement rather than valvuloplasty when the degree of regurgitation is more than moderate.
