Congenital adrenogenital syndrome is a disease caused by adrenocortical enzyme deficiency. A 10-year-old female with congenital adrenogenital syndrome was hospitalized for congestive heart failure caused by congenital mitral stenoinsufficiency. Coarctectomy and division of patent ductus arteriosus had been performed three months after birth. Repeated cardiac catheterization revealed severe mitral regurgitation, left ventricular dilatation, pulmonary hypertension, and high pulmonary wedge pressure. Incision of mitral papillary muscle and mitral annuloplasty at the posteromedian commissure were performed. The prolapsed anterior leaflet was suspended with artificial chorda made by expanded polytetrafluoroethylene suture. In this paper, we describe the perioperative and postoperative management of a patient with congenital adrenogenital syndrome.
