To establish a new in vitro endocrine therapy sensitivity test that enables use to easily and accurately predict the efficacy of endocrine therapeutic agents after breast cancer surgery, we assessed the usefulness of a collagen gel droplet embedded culture drug sensitivity test (CD-DST), a chemosensitivity test using collagen droplet culture. Using the endocrine therapeutic agents tamoxifen (TAM) and medroxyprogesterone acetate (MPA), the optimal conditions of drug exposure and the cut-off point of efficacy evaluation were determined. TAM was also used to determine the sensitivity of clinical materials by this method. Four human breast cancer cell lines [Estrogen receptor (ER)- and Progesterone receptor (PgR)-positive MCF-7 and ZR-75-1,and ER- and PgR-negative MRK-nu-1 and SK-Br-3] and clinical materials from 22 patients were examined in the tests. After exposure to TAM at 10^<-7> M, and MPA at 10^<-5> M, the Treated/Contorl (T/C)(%) for the receptor-positive cell line was significantly lower than that for the receptor-negative cell line (p<0.01). The conditions of TAM, and MPA exposure were set as continuous exposure at 10^<-7> M, 10^<-5> M, and the drug was considered "effective" at a T/C (%)≤60%. TAM, and MPA was evaluated as "effective" against the receptor-positive cells and "ineffective" against the receptor-negative cells. In patients who underwent postoperative adjuvant therapy with TAM, who were tested resistant to TAM by the CD-DST method tended to develop recurrence. These findings suggest that CD-DST can select appropriate endocrine therapeutic agents for breast cancer and predict their anticancer effects under optimal conditions of tumor exposure to TAM and MPA, and the cut-off point for efficiency evaluation.
