Vancomycin is a glycopeptide antibiotic that has bactericidal activity against a variety of gram-positive bacteria including methicillin-resistant staphylococci. A simple, rapid, specific, reversed-phase high-performance liquid chromatographic method for free-type vancomycin determination in serum was developed. The procedures involve pretreating the patient's serum by ultra filtration and injecting the filtrate into high-performance liquid chromatographic equipment. An analytical octadecylsilane column with 5 μm particle size was used and the eluent was detected at 240 nm. The standard curve was linear between 0 and 250 μg/ml, and the detectable limit was 2 ng at a signal-to-noise ratio of 3 : 1. The analytical time was about 7 minutes. The coefficient of variation of reproducibility was 0.578 -2.07% for the concentration range of 9.7-128.8 μg/ml, and the recovery rate was 94.5-104.9%. No interference was found with many currently used antibiotics. The serum concentrations in patients treated with vancomycin were monitored at regular intervals. The levels attained were considered therapeutically adequate for clinical improvements of these patients.
