To determine the serum concentration of clomipramine, a highly efficacious drug in the treatment of depression, we devised a simple, rapid method of determination using high-performance liquid chromatography with a high recovery rate using an 0DS.C_<18> pretreatment column instead of the conventional organic solvent extraction method. Detection was performed at a wavelength of 200 nm using a reversed-phase column for separation and with trimipramine as the internal standard substance. Other endogenous substances in serum or drugs administered in combination did not interfere with the measurement. Accuracy of measurement was satisfactory : the coefficient of variation of simultaneous reproducibility was 1.89-3.34% (X=43.7-416.3ng/ml) and recovery rate was 83-102%. Good linearity was also obtained (0-1000 ng/ml). This method, which is free from cross-reaction, facilitates specific determination in patients under multidrug therapy and is suitable for use in establishing the optimum dose and monitoring the blood concentration of the drug.
