<Case Report>Multiple lacunar infarctions of brain associated with the syndrome of inappropriate antidiuretic hormone (SIADH)

The patient was a 67-year-old male in whom multiple lacunar infarctions located in the cerebral white matter and pons were detected by means of the magnetic resonance imaging (MRI) technique. Following a convulsive seizure, hyponatremia, hypoosmolar serum and renal sodium wasting without dehydration or hypovolemia were exhibited and the antidiuretic hormone (ADH) level in the plasma was inappropriately high in relation to the hypoosmolar serum. Therefore, the clinical diagnosis of the syndrome of inappropriate antidiuretic hormone (SIADH) was established. Although it was suggested that SIADH was provoked by any of the lacunae, a particular SIADH-producing lesion in the brain was unable to be isolated.
