The antidepressants that are effective against depressive symptoms include mianserin, amoxapine, maprotiline, amitriptyline, imipramine, and its active metabolites such as nortriptyline and desipramine. We devised a new method of determining antidepressant levels in the blood, and established it as a routine test method prior to the implementation of therapeutic drug monitoring (TDM) for these various drugs. This method is high sensitive and highly specific, including high-performance liquid chromatography (HPLC) and simplified trace pretreatment. We applied this new method to clinical cases. As far as a solvent in the mobile phase was concerned, a mixture of phosphate buffer (containing 0.06% nonylamine), acetonitrile and methanol, was transferred by the isocratic method (at a rate of 0.8 ml/min, ambient), and trimipramine was detected at a wave length of 200 nm as the internal standard material. As pretreatment for serum specimens, sodium hydroxide was added to the serum to make it alkaline, followed by extraction with 1-chlorobutane (containing 1.5% isoamyl alcohol). Back extraction with hydrochloric acid was further performed, and the resultant solution was used, as it was, for HPLC. This method showed favorable linearity, reproducibility and had a recovery rate ; the time of analysis was about 17 min and the minimum limit of detection was 1 ng (S/N=3), calculated in terms of the amount of injection. When this method and fluorescence polarization immunoassay were compared regarding the total levels of the drugs, amitriptyline, nortriptyline, imipramine and desipramine, the coefficient of correlation was as favorable as 0.956. However, the levels obtained by the method tended to be slightly low. This method enables specific quantitative determination of each drug in patients undergoing simultaneous administration of at least two drugs, and the amount of each serum sample is less than that needed in the conventional methods for separation analysis. The simplified pretreatment involved in this method enables high sensitive determination. This method is therefore considered to be applicable for the routine administration and management of antidepressants, and medical examination with these drugs.
