This study was conducted to elucidate the mechanisms of T-lymphocyte subset abnormalities in dilated cardiomyopathy. T-lymphocyte subsets (CD3,CD4,CD8,CD4/CD8) were measured in 20 patients with dilated cardiomyopathy and in 20 patients with congestive heart failure caused by various heart diseases, as well as 24 patients with hypertrophic cardiomyopathy, 10 with acute myocardial infarction, 13 with healed myocardial infarction, 5 with acute myocarditis, and 133 normal control subjects. Cardiac index, left ventricular end diastolic dimension, and left ventricular fractional shortening were measured. CD4 and CD8 lymphocytes were 1198±6,697±4 in normal subjects, respectively. These were reduced to 776±38 (p<0.001), 479±24 (p<0.001) in heart disease patients as a whole. Reductions of CD4 and CD8 were most marked in patients with congestive heart failure, and was prominent in patients with dilated cardiomyopathy. CD3 and CD4 had positive correlations (r=0.96l, p<0.01 ; r=0.923,p<0.01,respectively) with cardiac index, and CD8 had a negative correlation (r=-0.885,p<0.01) with left ventricular end diastolic dimension, indicating that T-lymphocyte subset abnormalities are elicited by impaired cardiac function and that observed reductions in CD4 and CD8 in dilated cardiomyopathy are secondary to impaired cardiac function.
