<Case Report>Successful treatment of forced crying with levodopa in a case of vascular dementia

Forced crying appears in various neurologic diseases such as cerebro-vascular accidents, neuro-Behcet disease, amyotrophic lateral sclerosis, multiple sclerosis and brain tumors. Atropine, atropine-like substances, haloperidol, levodopa or amantadine were reported as effective drugs for this symptom. But there has been no certain evidence which drug is most effective. We report here on a man with severe degree of forced crying who suffered from vascular dementia and moderate degree of normal pressure hydrocephalus. After the administration of levodopa, the patient showed considerable improvement in intensity, duration and frequency of his forced crying. This fact strongly supports the hypothesis by Udaka et al. that forced crying might result from the hypofunction of dopaminergic neuron in the brain.
