<Originals>Night-and-Morning Variation of Plasma Cortisol Concentrations in Healthy Subjects and Patients with Cushing's Syndrome

A simple and reliable test to diagnose Cushing's syndrome by utilizing continuous observation of plasma cortisol concentrations for a 12 h pericd, from 2000 h to 800 h next morning, was presented. In the study of healthy subjects, it was revealed that the normal circadian variation in plasma cortisol levels was characterized by a pattern with a nadir at 100 h followed by gradual increase to a sharp peak at 500-600 h. On the other hand, two patients with Cushing's syndrome due to an adrenal adenoma and bilateral hyperplasia respectively did not exhibit such diurnal rhythmicity in plasma cortisol as observed in normals. Namely, both of the patients did not show any appreciable nadir or peak in cortisol levels during the observation period. It was concluded that monitoring of 12 h circadian variation in cortisol levels is useful for the differentiation of Cushing's syndrome from normalcy.
