An initial series of patients who were treated with endoscopic sphincterotomy (EST) for choledocholithiasis and papillary stenosis was reviewed. EST was attempted in 18 patients and was successful in 17 (94.4%). Of 12 patients with choledocholithiasis (6 with impacted stones) basket extraction was attempted immediately after EST in 5 patients and 3 to 10 days later in 4,and was successful in all. Four of the 10 patients with gallbladder in situ underwent cholecystectomy one to three weeks after EST. In 3 patients with papillary stenosis EST was successfully performed to unroof the narrow distal segment. EST was also utilized as a preparatory procedure for the endoscopic introduction of biliary prosthesis in 2 patients with obstructive jaundice. Four patients developed immediate complications such as bleeding, acute pancreatitis and possible perforation, but responded favorably to conservative management. Follow-up studies from one month to 3 years revealed no late complications. EST has proved to be an effective and relatively safe method for the treatment of choledocholithiasis and papillary stenosis.
