<Originals>Aggravating factors, clinical course and prognosis in cases of acute hepatic failure on chronic liver disease

In 20 cases of acute hepatic failure on chronic liver disease, the relationship of aggravating factors between the clinical course and prognosis was studied. The patients were divided into two groups : one of 8 cases of the acute-on-chronic type (Group I), which was considered to be caused by a new factor, and the other group of 12 cases of the terminal coma-like type of cirrhotic encephalopathy (coma-like type ; Group II), which was thought to be caused by acute exacerbation of the underlying liver disease. The subjects were 16 male and 4 female patients, and most of them were in their forties. The aggravating factors were alcohol, drugs, blood transfusion, infection and congestive heart failure in Group I (acute-on-chronic type), whereas they were surgical operation and digestive tract hemorrhage in Group II (coma-like type). In Group I (acute-on-chronic type), the clinical course was usually slow, and 3 cases out of the 8 survived or lived long, indicating relatively good prognosis. On the other hand, the prognosis was poor in Group II (coma-like type), and all patients, who developed encephalopathy within 2 days died in a short time. Six out of the 8 cases autopsied in Group I and II, massive or submassive hepatic necrosis was observed.
