It is most useful that the introduction of a panendoscope not only in emergency but also in routine examination includes visualization of the esophagus, stomach and duodenal bulb at one time. Between Dec. 1974 and Feb. 1978,panendoscopy using the GIF-K was employed as a routine endoscopic procedure in 821 consecutive patents who consulted the doctors of Mohri Hospital for Gastrointestinal Diseases. The GIF-K is the oblique viewing upper gastrointestinal panendoscope whose distal end has an angle of view direction oblique 30 degrees from longitudinal axis. The cardia and duodenal bulb are more easily visualized by the GIF-K than the GIF-D2,another forward-viewing panendoscope. But the GIF-K has weak points that there are some blind areas at the angulus of the stomach. Then, the GIF-K must be reformed to make its shaft slender and flexible in that point of view. Routine panendoscopy was opened to examine the esophageal lesions, especially the esophageal early cancer. The diagnostic yield was higher in the panendoscopy group than in the radiological group in small mucosal lesions. Panendoscopy gave the consecutive diagnosis of the upper gastrointestinal tract and recognized the combination and interference of the esophagus, stomach and duodenal bulb.
