We encountered a case of mefenamic acid ingestion resulting in grand mal seizures. After a quarrel with his girl friend, a 25-year-old male ingested 90 capsules of Pontal, a commercially available antiinflammatory analgesic (22.5g as mefenamic acid). He developed disturbance of consciousness and was admitted to this hospital as an emergency case. At the time of admission, the patient's level of consciousness was 200 on the Japan Coma Scale and grand mal seizures were also noted. The seizures subsided after intravenous administration of diazepam, but marked metabolic acidosis was observed. Gastric lavage was carried out immediately after endotracheal intubation. Blood purification therapy was then given by a direct blood adsorber and a hemodialyzer connected in series. Level of consciousness improved after blood purification therapy, allowing removal of the tube.
