<Case Report>Paramnesia and Schizophrenia-Like Symptoms in a Hyperthyroid Patient after Thyroidectomy

A 23-year-old hyperthyroid female patient suddenly developed schizophrenia-like symptoms such as delusions, and hallucinations after subtotal thyroidectomy. Though the thyroid hormone level in the circulating blood returned to the normal range after operation, the patient's typical symptoms strongly suggested thyroid crisis. Careful observation of extraordinary experiences of the patient revealed not only schizophrenia-like symptoms but also paramnesia. The latter was one of the pathological conditions of the memory, in which the present world is mistaken for the past world. ^4 Delusions or hallucinations of the patient represented a marked trend of her desire itself. Moreover the content of them was strongly affected by the paramnesia mentioned above. After these psychotic symptoms subsided, personality change^<17> which was akin to so-called "Defektzustand" in German medical term persisted. ^<3,11> Also it must be stressed that the genetic factors which were undoubtedly found in the patient's family seemed to play an important role in the development of these psychotic symptoms.
