• Resistance to antifungal agents, especially azole agents such as fluconazole, in the opportunistic yeast Candida albicans is increasing in the U.S.A. and Europe. We studied the present state of antifungal resistance in our hospital and the development and potential mechanisms of resistance. The antifungal susceptibility of 285 C. albicans strains isolated over the last two years was investigated using the broth microdilution method. Most of the isolates showed relatively low MIC values in all five agents studied. However, two resistant strains to flucytosine and one resistant strain to fluconazole were found. The latter showed cross resistance to miconazole and itraconazole. In vitro induction of resistance with fluconazole was attempted. The four fluconazol susceptible strains were passaged in liquid yeast nitrogen base medium containing fluconazole. Fluconazole resistance developed in two out of four strains after serial passages. The length of time required for resistant induction differed between the two strains. Moreover, quantitative analysis of mRNA using reverse transcription polymerase chain reaction revealed increased mRNA levels of BEN^r gene, which codes drug efflux pump, in induced resistant strains derived from 95-77. The mRNA levels of BEN^r gene in 96-579,in which trailing phenomenon was clearly seen in the susceptibility testing, were high even in parent strain. These findings indicate that emergence of azole resistance is related to continuous antifungal exposure, and that the ability for acquisition of resistance differs among strains.
