Immunological Mechanisms of Cytotoxicity in Patients with Chronic Glomerulonephritis

Immunological mechanisms of cytotoxicity in renal biopsy specimens were evaluated in twelve patients with various types of chronic glomerulonephritis. These mechanisms included: (1) antibody and complement (Ab+C), (2) cell-mediated lympholysis (CML), (3) antibody dependent cell-mediated cytotoxicity (ADCC), and (4) antibody, polymorphonuclear cells and complement (PMN+C). The majority of the patients, except for the minimal change type of chronic glomerulonephritis, showed positive cytotoxicity mediated by Ab+C. Certain patients with advanced stages of chronic proliferative glomerulonephritis demonstrated cytotoxicity due to both Ab+C and PMN+C. None of the patients examined showed cytotoxicity due to CML or ADCC. Patients with the minimal change type of chronic glomerulonephritis did not show any one of the four different mechanisms of cytotoxicity. This is the first report giving direct evidence indicating that complement activated by the immune complex is the major cause of tissue damage in patients with chronic glomerulonephritis.
