Studies on Insulin Binding Activity of Diabetic Neutrophils

It has been postulated that the cells from diabetic patients show less sensitivity to endogenous insulin than those from healthy subjects. The purpose of this study was to investigate the mechanisms inducing insensitivity to insulin in diabetic patients. Two parameters were employed to examine such mechanisms: the first was the assay of insulin binding activity and the second was the measurement of intracellular metabolic processes. The insulin binding activity, ^<14>CO_2 production of neutrophils, in vivo index of insulin sensitivity (Kitt) and the glucose tolerance test were measured in 20 diabetic patients and in five healthy adults. The conversion rate of ^<14>C-glucose to ^<14>CO_2 in control neutrophils was more than twice that in diabetic neutrophils. There was a significant correlation between Kitt and ^<14>CO_2 production (r=0.80, p<0.01), as well as Kitt and insulin binding activity (r=0.59, p<0.01). These results suggest that insensitivity to insulin observed in diabetic patients may be due to intracellular metabolism rather than the decrease in insulin binding activity.
