Interrelationship between portal vein pressure and portal systemic shunt in patients with liver disease.

Portal vein pressure (PVP) and sum of the diameters of collateral routes (total collateral circulation) were measured in 155 patients with liver disease by percutaneous transhepatic portography. Furthermore, "portal vein shunt index" was evaluated in 100 cases, "splenic vein shunt index" in 50 cases, "total (portal systemic) shunt index" in 52 cases using macroaggregated albumin labeled with different radionuclides, The results are as follows. 1) As the "total collateral circulation" became large, "total shunt index" increased. 2) "Total collateral circulation" increased with elevation of portal vein pressure, but with very large "total collateral circulation", PVP was often low. 3) The correlations between PVP (Y) and total (portal systemic) shunt index (X), splenic vein shunt index (X), and portal vein shunt index (X) were expressed best as Y=-0.070XX<SUP>2</SUP>+7.63X+180.91 (N=52, r=0.7494, p<0.001), Y=-0.065XX<SUP>2</SUP>+6.31X+241.27 (N=50, r=0.6269, p<0.001), and Y=-0.047XX<SUP>2</SUP>+4.46X+252.59 (N=100, r=0.4037, p<0.001), respectively. Increasing portal vein pressure were associated with progressively higher indices of portal vein shunt, splenic vein shunt, and total (portal systemic) shunt up to a certain level. Beyond this level, portal vein pressure did not increase further, and rather it decreased somewhat with further increasing degrees of splenic vein shunt, and total (portal systemic) shunt. These results indicate that portal systemic shunt keeps portal vein pressure within certain limits or decreases it.
