Estrogen-like activity in Kampo medicines used for menopausal symptoms and gynecological diseases

漢方薬は, 更年期症候群の種々の症状を緩和するためにホルモン補充療法(HRT)の代替療法として用いられてきた。しかし漢方薬自身にエストロゲン様活性があるか否かについては明確にされていなかった。我々は, 漢方薬のエストロゲン様活性を評価するために, MCF-7細胞を用いた高感度ルシフェラーゼレポーター遺伝子アッセイを確立した。更年期症候群および各種婦人科疾患の治療に使用される漢方薬25種についてそのエストロゲン様活性を測定した結果, 当帰芍薬散(TJ-23)を除く24種の漢方薬にエストロゲン様活性が検出された。漢方薬1日投与量あたりのエストロゲン様活性をエストラジオール(E2)量として換算したところ, 葛根湯(OMRC-K1), 葛根湯(TJ-1)及び葛根紅花湯(OMRC-K2)のエストロゲン様活性はそれぞれ3.2, 1.5, 及び1.0μgであり, 他の漢方薬の活性は150ng以下と予想外に低い値を示した。これらの24種の漢方薬のエストロゲン様活性は, 西洋医学のHRTに用いられるエストロゲン量(625μg/日)と比べても極めて低かった。次に, これらの漢方薬中で最も活性の高かった葛根湯(OMRC-K1)のエストロゲン様活性発現機構を調べた結果, エストロゲンと同様にエストロゲン受容体及びエストロゲン応答配列を介して活性発現していることがわかった。以上の結果から, 漢方薬はエストロゲン様活性を有するが, その活性は低いため, 他の作用機序を介して臨床効果を発揮している可能性があり, HRTに比べて安全性が高いであろうことが予想された。今後, 漢方薬の長期投与の安全性を確保するために, 漢方薬投与後の血中のエストロゲン様活性の測定及び, ホルモン感受性癌に対する効果の解析等の検討を行う必要がある。
