Combination of Recombinant Human BMP-2 and Atelocollagen Implant with Cortical Perforations for Bone Augmentation

The purpose of this study was to determine whether bone augmentation could be obtained by the composite of recombinant human bone morphogenetic protein-2(rhBMP-2)and bioabsorbable atelocollagen, and to compare the efficacy of cortical perforations. The onlay implant was inserted into the space beneath the periosteum or the galea aponeurotica of 10-month old rats. A combination of the rhBMP-2/collagen subperiosteal implant with cortical perforations resulted in an initial acceleration of osteogenesis and the excellent augmented bone was connected directly with the original bone and exposed marrow space, whereas the rhBMP-2/collagen without perforations resulted in peripheral bone with many round acellular spaces in the middle area at initial stages. In subgaleal group, the rhBMP-2/collagen resulted in osteoblasts differentiation under the galea at 1 week. The carrier atelocollagen was completely replaced by bone tissue in the presence of rhBMP-2. Our present results indicate that the combination of cortical perforations to host bed and the rhBMP-2/collagen implant could accomplish remarkably success for bone augmentation. The rhBMP-2/collagen onlay implant may be clinically applicable even under the condition without the periosteum.
