Adult Onset Juvenile Rheumatoid Arthritis : Febrile Episodes Controllable at Out-Patient Clinic

Episodic high fever and generalized skin eruptions of adult onset of juvenile rheumatoid arthritis was observed in a 56 year-old Japanese male patient. The onset of the same attacks started at his age of 26. For the last 30 years, he had experienced eleven times of episodes with fever and skin eruptions. High-dose corticosteroid therapy was proven to be most effective on the improvements of clinical and laboratory abnormalities, including elevated titers of ESR, C-reactive protein, GOT, GPT, alkaline phosphatase, LDH, fibriogen, C_3 and C_4, in association with leukocytosis and elevated serum immunoglobulin levels. Considering patient's quality of life (QOL), it could be important for physicians to control disease activities as much as possible at the out-patient clinic without hospitalization.
