Arthritis Caused by Chlamydia in Reiter Syndrome : A Case Report of Long-term Follow Up

We have identified Chlamydia in the synovial tissue of a patient with Reiter syndrome, and we followed up this patient for 15 years long. At the initial visit the patient showed severe flexion contrcture of the left knee. Total synovectomy had been carried out. After the synovectomy the patient was improved and the flexion contracture resolved completely. The synovial tissue obtained at the synovectomy showed thickening of the lining cells of the synovial membrane and marked perivascular mononuclear cell infiltration with small lymphocytes, plasma cells, and macrophages. And sections treated with fluorescein-conjugated monoclonal anti-Chlamydia antibody showed intracellular Chlamydia in the synovial tissue. The patient had often experienced the recurrence of arthritis of ankle joints since the first treatment. However, radiographic deterioration was not remarkable on both knees and ankles. Thus the prognosis of our patient with Reiter syndrome was good. Therefore it is important for individual patient to know that severe destructive disease is extremely rare as demonstrated in our case.
