Total Hip Arthroplasty with Chiari Pelvic Osteotomy in Dysplastic Hips

Reconstruction of severe dysplastic hip in adulthood is still a major task of total hip arthroplasty (THA). Previous studies demonstrated high rates of complications and revisions of THA in this condition. Nine cases (10 hips) with congenital dislocation or dysplasia underwent THA. The acetabula were reconstructed with Chiari pelvic osteotomy simultaneously. They were followed for an average of 4.6 years (range; 2.5-10.0 years). The average of clinical hip scores was elevated from 48.4 to 81.4 points by the surgery (Japanese Orthopaedic Association Hip Score). Individual scores were as follows: pain; 16.0 to 37.8 points, gait; 8.5 to 13.6 points, range of motion; 13.4 to 16.6 points; activity of daily living; 10.5 to 13.6 points. Radiographical loosening of the socket was not evident in any case except for one patient. In conclusion, Chiari pelvic osteotomy provides acetabular coverage to fix the socket. It is, thus, recommended for patients with these conditions at the time of THA.
