Abnormally Shaped-Polyclonal Hypergammaglobulinemia in Pulmonary Fibrosis, Splenomegaly, Sjogren Syndrome and Reactive Lymphadenopathy

A 73 year old female is presented. She started her clinical complications of leucopenia, splenomegaly, and chest X-ray abnormality of granular-reticular markings at the age of 55. She was found to have an abnormally-shaped hypergamma-globulinemia, mild proliferations of plasma cells in repeated bone marrow aspiration (2.0, 3.2, 5.0%, out of nucleated cells in the marrow), elevated IgA (2,550 〜 4,852 mg/dl), and elevated IgG (3,290 〜 3,486 mg/dl). However, repeated analyses by immunoelectrophoresis as well as immunofixation could not identify any evidence of monoclonal immunoglobulins in the serum or Bence Jones proteins in the urine. Clinical diagnosis varied along the course such as Banti syndrome, pulmonary fibrosis, Sjogren syndrome and malignant lymphoma. After 18 years' clinical course, she died of lymphoadenopathy and hemorrhage. Autopsy findings revealed no evidence of malignant lymphoma. Detailed clinical findings were discussed in this paper.
