The relation between diabetic retinopathy and α_2M level was investigated. Since α_2M is variable according to age and sex, matched controls were selected from among the diabetic control and healthy control groups. The relation between degree of retinopathy and level of α_2M was such that α_2M generally decreased as the degree of retinopathy became exacerbated. However, the α_2M in case of proliferative retinopathy increased significantly when positive proteinuria was present. It is concluded therefore that the level of α_2M in diabetes mellitus may vary according to the kind of diabetic complications, and that either an increase or a decrease in α_2M may affect the development of micro- and macroangiopathy in diabetes through the action of antiplasmin or antithrombin.
