Studies on the Zone Phenomenon in the Determination of Immunoglobulins by Laser Nephelometric Immunoassay

We studied the zone phenomenon in the determination of immunoglobulins (IgG, IgA and IgM) by laser nephelometric immunoassay and obtained the following conclusion. The phenomenon could not be observed in IgG even at the concentration of 9765 mg/dl. In the case of IgA, the zone phenomenon due to excess antigen was found. In 15 out of 16 such cases, both serum IgG and IgM levels were abnormally low. Therefore, when an M-protein is present, we have to suspect and confirm the zone phenomenon by performing cellulose acetate electrophoresis or the addition test in which a known quantity of immunoglobulin is added into the serum determined. In the case of IgM, there was no distinct change in serum IgG, IgA and total protein levels. Therefore, we could not know whether the abnormally low levels of IgM was due to the zone phenomenon or not. All of these cases were found in patients with primary macrcglobulinemia. In order to detect the zone pheuomenon in the IgM determination, the informations on primary macroglobulinemia from clinical doctors are very helpful.
