IMMUNOHISTOCHEMICAL IDENTIFICATION OF KERATIN, INVOLUCRIN, AND EPITHELIAL MEMBRANE ANTIGEN IN PAGET'S DISEASE.

Immunohistochemical profiles of keratin, involucrin, and epithelial membrane antigen (EMA) are described for lesions of Paget's disease including normal skin and skin appendages by using monoclonal antibodies and conventional polyclonal antiserum. In normal epidermis, basal cells were stained strongly by PKK1 monoclonal antibody (detecting 44, 46, 52 and 54 kDa keratins), and spinous and upper layer cells gave a positive reaction with KL1 monoclonal antibody (55-57 kDa). Involucrin was expressed in upper spinous cells, but not in basal ones. EMA staining was negative in normal epidermis. Secretory epithelial cells of eccrine and apocrine glands displayed positive reactions with the two monoclonal antibodies, to keratin (PKK1 and KL1). Paget cells were characterized by intense staining with PKK1 and KL1 keratin and EMA. Involucrin staining in Paget cells was negative. It is suggested that Paget cells may originate from the secretory cells of sweat glands.
