Reinforcing Effect of the New Synthetic Bioabsorbable Sealant (ADVASEAL^<TM>) for Bronchial Stumps after Pulmonary Resections in Pigs

Postoperative bronchial fistulas are classified as early or late. The cause of early fistula is thought to be related to technical problems in the treatment of the bronchial stump. To prevent early bronchial fistula, it is necessary to reinforce the bronchial stump. We examined the reinforcing effect of the new synthetic bioabsorbable sealant (ADVASEAL^<TM>) for the bronchial stump after lobectomy in 10 pigs. The animals were anesthetized and underwent right-side anterolateral thoracotomy for resection of the cranial lobe. The cranial bronchus was resected by an autosuture. The pigs were assigned to either the control group (n=5) or the reinforcing group (n=5). The solution used for reinforcement of the bronchial stump was ADVASEAL^<TM>. Controls received no reinforcing. After lobectomy, we studied the rupture pressures of the bronchial stumps. Those of the reinforcing group were greater than those of the control group (p<0.05). This result suggests that ADVASEAL^<TM> effectively reinforces for the bronchial stump. It is concluded that sealing the bronchial stump with ADVASEAL^<TM> may prevent early stage bronchial fistulas.
