SUCCESSFUL CLINICAL APPLICATION OF TISSUE ENGINEERED BLOOD VESSEL

Currently used prosthetic or bioprosthetic materials lack growth potential and, therefore, subsequently require replacement in pediatric patients as they mature. Tissue engineering (TE) is a new discipline that offers the potential to create replacement structures from autologous cells and biodegradable polymer scaffolds. In May 2000 we started the clinical application of tissue engineered vascular grafts. Until October 2001,nineteen patients underwent TE graft implantation. The patients' parents were thoroughly informed and consented to the procedure. An approximately 2-cm segment of peripheral veins were explanted and cells from its walls isolated. Cells were cultured and expanded. A polymer tube that served as a scaffold for these cells was composed of a copolymer of PCL-PLA (50 : 50) reinforced with woven PGA. The biodegradable polymer conduit was designed to degrade within 8 weeks. Ten days after seeding, the graft transplantation was performed. No postoperative complications occurred. At 17 months after implantation, the first patient was doing well with no evidence of graft occlusion or aneurysmal changes on chest roentgenogram. Long-term follow up is obviously necessary to confirm the feasibility of this approach. Because both our laboratory and clinical experiences are quite encouraging, we suggest that the TE approach may play an important role as an alternative method to transplantation and to the use of artificial organs in the field of pediatric cardiovascular surgery.
