Problems associated with the ultrasonic (US) diagnosis of small hepatocellular carcinoma (HCC) was studied on 119 cases, especially on 31 cases with small lesions under 3cm in diameter. Ultrasonography was useful for the screening of small HCC, but there were not a few undetectable cases. It is difficult to make a qualitative diagnosis of small lesions under 2cm in diameter. And echogenic nodules accounted for 32% of the nodules of hepatoma, so care should be taken with the US method to differentiate echogenic small HCC, from other echogenic lesions.Detectability was compared between the US method and angiography on 25 cases with lesions of less than 3cm, diameter, in 64 nodules. It seems to be difficult to detect small lesions under 1cm in diameter by the US equipment available presently. Cases were rendered undetectable by the dead zone cused by the portion of the dome of the right hemidiaphragm, and also due to distorted intrahepatic echograms, liver atrophy and liver deformity arising from hepatic cirrhosis.
