Biomarkers of endothelial dysfunction are elevated in patients with rheumatoid arthritis with oketsu (blood stasis)

• 先に我々は〓血病態と関節リウマチ(RA)の活動性との関連性について報告した。近年,関節リウマチ(RA)患者の生命予後を悪化させる要因として,動脈硬化病変が注目されるとともに,動脈硬化症の前段階として認められる血管内皮障害が炎症に基づき生じ,RA患者に合併する心血管疾患にも密接に関与することが示されている。今回,我々はRA患者における〓血病態と血管内皮機能障害を示唆する血中マーカーとの関連性について検討した。対象は少なくとも3ヶ月間,投与薬剤の変更がない50例のRA患者で〓血スコアにより〓血群25例と非〓血群25例に分けた。RAの活動性,炎症マーカーとしての好中球エラスターゼ,凝固系マーカーを測定し,さらに血管内皮機能障害の血中マーカーとしてトロンボモジュリン,von Willebrand因子,t-PA,PAI-1,sE-セレクチン,sICAM-1,sVCAM-1を測定し2群間で比較した。両群の臨床背景や疾患活動性は骨変形度以外に差は認めなかった。t-PA,sICAM-1,sVCAM-1は〓血群が非〓血群に比べて有意に高い値を示した。〓血病態を有するRA患者では血管内皮障害を示唆する因子の上昇を認めたことから,RA患者において〓血病態はより動脈硬化への進展が促進されやすい状態にあると考えられた。
