Changes in renal osteodystrophy and skeletal complications following renal transplantation in 14 children were analyzed. Osteoporosis was found in almost all patients after renal transplantation, and a pathological fracture was recognized in one patient. The main findings of rickets and secondary hyperparathyroidism disappeared or improved within 3 years after renal transplantation, and the values of serum alkaline-phosphatase and C-terminal parathyroid hormone decreased rapidly following renal transplantation. In this way, renal transplantation is considered to be a useful way to improve renal osteodystrophy. However, caution should be exercised in patients with osteoporosis given large dosage of corticosteroid. The possibility of aseptic osteonecrosis also remains although we did not detect such a complication in our cases.
