Nephrotic syndrome after Henoch-Schonlein purpura generally carries a good prognosis. Renal failure among this group of patients is rarely observed. We reviewed all cases of Henoch-Schonlein purpura with renal involvement presenting as nephrotic syndrome during the past ten years (1976-1985) from different institutions in Japan. From a total of 54 patients, 25.9% (14/54) went into renal failure. The likelihood of going into renal failure is further increased when nephrotic syndrome is associated with renal insufficiency during the first three months of onset.
