Kidney Transplant Nephrotic Syndrome with Minimal Histologic Change in Focal Segmental Glomerulosclerosis: Clinical Recurrence?

A 8-year-old girl of focal segmental glomerulosclerosis had a live-related donor kidney transplantation from her mother. Recurrent nephrotic syndrome was found 1 month after renal transplantation. Light microscopy showed minimal histological change while immunofluorescence microscopy revealed no significant stain for immunoglobulin and complement, electron microscopy demonstrated fusion of foot process. Progressive renal dysfunction was observed 12 months after transplantation which was attributed to chronic rejection with ciclosporine A nephrotoxicity. At present she is being maintained with small doses of multiple immunosuppressants, clearance of creatinine is about 30ml/min/1.73 m^2.
