We studied two patients with primary distal renal tubular acidosis who were diagnosed during infancy and childhood and followed for 20 and 7 years, respectively. Acute furosemide loading test showed the inability to lower urine pH maximally accompanied by a marked increase in urinary potassium excretion suggesting the presence of proton pump defect. To assess the capacity of the distal tubule for sodium reabsorption, lithium clearance study was done which showed unimpaired distal sodium conservation. Our result is contrary to previous reports of sodium wastage and probably a reflection of the heterogeneity of the underlying defect in primary childhood renal tubular acidosis-1.
