26 Year Follow up of Childhood Distal Renal Tubular Acidosis

Primary distal renal tubular acidosis (RTA) was diagnosed at 2 months of age in a female child. Bilateral nephrocalcinosis was detected radiologically at this time. She was started on daily alkali supplements at 4mEq/kg/day which she continued to take for 26 years. Her serum bicarbonate levels during follow up varied from 18-26 mmol/l. Radiological evaluation revealed no rickets. Her height showed an initial acceleration but remained at about -2 SD throughout childhood. Her height at 26 years is 138cm which is at -3.4 SD. Nephrorcalcinosis remains unchanged. Her creatinine clearance is 53.1ml/min/1.73m^2. This suggests that despite early introduction of alkali therapy at 4 mEq/kg/day stunting of adult height is not prevented. In patients with distal RTA with nephrocalcinosis renal function may deteriorate in early adulthood despite continuous alkali therapy and apparent nonprogression of nephrocalcinosis radiologically.
