Growth Velocity and Nutritional Status in Young Children Undergoing Continuous Ambulatory Peritoneal Dialysis

Growth velocity and nutritional status of seven young children (1 to 7 years old) undergoing CAPD was studied. All the patients were understatued (<-2.5SD) before starting of CAPD. During the observation period for 8 to 27 months, 4 patients achieved more than 80% of expected growth velocity, and 3 were less than 80% of expected growth velocity. Growth velocity of one patient who achieved marked improvement of percent growth velocity after starting of CAPD(59 to 126%), declined again after discontinuing of CAPD. Between these groups which achieved good growth velocity and poor growth velocity, there was marked difference of mean dietary energy intake of 101 kcal/kg/day in the good growth velocity group and 79 kcal/kg/day in the poor group, (p<0.001) and mean protein intake of 3.9g/kg/day in the good group and 3.1 g/kg/day in the poor group, (p<0.001). Several plasma amino acid data such as valine to glycine ratio, tyrosine to phenylalanine ratio, cystine levels showed significant difference between two groups, and better controlled uremic status and nutritional status in the group of good growth velocity was suggested. From our data, CAPD seems to be a good technique for management of young uremic children complicated with poor growth if sufficient energy and protein intake is maintained.
