Recurrence and Rejection after 14 Years of Post-transplant Course in Henoch-Schonlein Purpura Nephritis (HSPN) : A Case Report

A 19-year-old male underwent renal transplantation when he was 4 years and 6 months old after having an inexorable course of HSPN. His clinical course was followed-up for 14 years and was characterized by a fairly stable course. Progressive deterioration of kidney function was seen following a three month period of medical interruption. Recurrence of the original disease possibly coexisting with chronic rejection process was suggested by the immunopathologic finding in the donor kidney. Non-compliance in medication, an important phenomenon which can often be neglected, should always be anticipated among longterm survivors of kidney transplant.
