Laparoscopic Heminephrectomy of Horseshoe Kidney : A Technique of Extracorporeal Isthmus Transection

Laparoscopic surgery provides a minimum invasive alternatives to an open operation. We report a case of laparoscopic heminephrectomy for a non-functioning giant hydronephrosis found in a horseshoe kidney. A 61-year-old housewife was referred to our hospital because of a large abdominal mass. Imaging studies revealed a horseshoe kidney associated with hydronephrotic right hemisphere. The resected specimen weighed 480gm and the hydronephrotic pelvis measured 16×11cm in diameter. The total operating time was 270 minutes. Estimated blood loss was 240ml and no blood transfusion was required. Postoperative course was uneventful and the patient was discharged from the hospital on the 9th day following surgery. To our knowledge, this is the first case of laparoscopic heminephrectomy of horseshoe kidney associated with right giant non-functioning hydronephrosis. Though challenging, the evolution of laparoscopic surgery continues to change the approach to surgical treatment of certain urological diseases.
