Early Complications and Morbidity after Radical Prostatectomy in Japan : The Kitasato Experience

Radical prostatectomy has gained increasing popularity as a curative treatment option for clinically localized prostate cancer. Its efficacy, however, conies at the price of morbidity. Hospital records of 154 Japanese patients who underwent bilateral pelvic lymph node dissection and radical prostatectomy for clinically resectable prostate cancer were reviewed to document the adverse events that occurred within 30 days following operation. Risk factors for perioperative complications were analyzed by multivariate logistic regression analysis. Fifty patients (32.5%) suffered at least one complication perioperatively. Two patients (1.3%) had life threatening complications that required intensive treatment or major surgical intervention. The most frequent major early postoperative complication was wound infection (5.2%). No patients had clinically recognized thromboembolic complications. Overall, the operating time averaged 222.5 minutes. Blood loss averaged 1,697.3ml. Both these parameters decreased steadily with time, indicating a typical learning curve. The hospital stay averaged 24 days and decreased significantly with time. There were no deaths within 30 days following operation. After controlling for age, preoperative morbidity, preoperative hormonal manipulation and intraoperative blood loss, operating time was the only independent covariate significantly associated with an increased risk of major complications (p=0.04). Radical prostatectomy for prostate cancer is major urologic surgery, with a substantial associated risk of morbidity. A learning curve clearly exists, however, and the operation can be carried out with acceptable morbidity when it is managed by experienced surgeons. Further efforts should be focused on decreasing major complications and reducing morbidity, and on studying resource economy for future policy establishment.
