Long Term Follow up of Postoperative Intravesical Instillation Therapy for Superficial Bladder Carcinoma

We reported the prophylactic effect of post transurethral resection (TUR) intravesical instillation therapy for superficial (stage T_a and T_1) bladder carcinoma. We report here a long term prophylactic effect of this intravesical instillation therapy. Diluted and non-irritative anti-tumor solution was instillated intravesically for three consecutive days after TUR in 69 superficial bladder carcinoma patients to prevent potential dissemination of tumor cells and their implantation in the bladder wall. Mitomycin-C (27 patients), bleomycin (22 patients) and adriamycin (20 patients) were used for this study. The non-recurrence rates of bladder tumors in five years were 52% in mitomycin-C, 54% in bleomycin and 46% in adriamycin, respectively, whereas there were no statistical significance among three groups (generalized Wilcoxon test). In two cases of adriamycin group, the instillation therapy had to be discontinued because of a remarkable postoperative hemorrhage. None of the patients complained of symptoms of bladder irritation with these diluted anti-tumor solutions. The results with this technique are highly encouraging in reducing the recurrence rates of superficial bladder tumor after transurethral resection.
