The first fifty stress urinary incontinent patients underwent suspension of bladder neck by a modified Gittes procedure during the period between May, 1989 and October, 1990. Our modifications are a pair of small incisions on the suprapubic region and quantifying thread tension by using a spring scale. Their ages ranged from 31 to 74 years old with a mean age of 53.5 years old. They were followed for a minimum of 6 months to 23 months. Some stress incontinence recurred in five patients (10%) within 4 months postoperatively. Three (6%) were symptomatic enough to remove the sutures: two patients complained of persistent lower abdominal traction pain and one suffered from late infection. Overall success rate was 84% (42/50). This modified procedure is timesaving, less traumatic and easy to perform.
