The difference in therapeutic effect of transurethral microwave thermotherapy between prostate cancer and benign prostatic hyperplasia

Background : The effect of transurethral microwave thermotherapy (TUMT) for prostate cancer has been thought to be insufficient because of limitation in reaching the peripheral prostate. In the present study, we employed androgen deprivation therapy (ADT) for 3 months prior to beginning the TUMT, which made the thermotherapy significantly more effective due to the reduction in prostate volume. Materials and Methods : Thirty patients with clinical stages T1-T2N0M0 were treated with TUMT from December 2001 to April 2004. The therapy started with ADT using luteinizing hormone-releasing hormone (LH-RH) agonist and nonsteroidal antiandrogen for 3 months prior to high-energy TUMT using the UroWave (Dornier MedTech Systems GmbH). Radical transurethral resection of the prostate (TURP) was performed at least 3 months after TUMT with the UroWave and all the TURP chips were examined to confirm the treatment efficacy. Results : Significant reduction in volume of the prostate (mean 36%) was noted after ADT for 3 months. The prostate volume at the time of TUMT was small enough (mean 25.5 ml) to be thoroughly treated with high-energy microwave to the peripheral prostate. All the TURP chips were submitted to thorough pathological examination, which revealed no cancer in 26 of 30 patients, whereas 13 patients revealed active remnants of benign prostatic hyperplasia (BPH). Conclusions : The combination of ADT and TUMT is an effective therapy for localized prostate cancer.
